Application Form

To Download File:

1. Right Click on PDF Icon above

2. Click Download Linked File

3. The file will be in Downloads Folder on Your Computer

OR

1. Left Click on Icon Above

2. Go to File

3. Click Save As

4. Rename File and select Save Location

5. Hit Ok

6. Find Your File at the Specified Safed Location on Your Computer

OR

1. Click on Icon

2. PDF will Open in Separate Tab

3. Fill Out Form

4. Print then Sign and Date.

Download the application, fill it out, sign and date. Then email back to info@ugotitamigos.org along with a copy of your DD 214
